Palliative care improves the quality of life for patients with a chronic or serious illness. This type of care focuses on providing relief from symptoms and stress associated with the illness. We collaborate with your healthcare provider to develop a personalized care plan.
Unlike hospice which focuses on the final stages of life, palliative care supports your care needs during all stages of a serious illness.
Supportive care during a serious illness
Navigating a hospital stay can be unfamiliar and daunting. Our palliative care services reduce hospitalizations and allow patients to stay in their familiar surroundings. Our certified nurse practitioner coordinates a plan of care in collaboration with your primary medical providers and works with the patient and family to understand the goals of treatment.
Your care team supports not only your loved one, but also your family. We educate families on what to expect at each stage of the illness, encouraging healthy discussions around advanced directives, and providing counseling and connections to additional supportive resources.
