Through pain and symptom management, we prioritize the safety and comfort of patients in their familiar surroundings and support families throughout this critical time.
Circle of Care
You don’t have to make these decisions on your own. A multidisciplinary team of professionals, specializing in end-of-life care and dedicated to your loved one’s needs offers medical, emotional, social, and spiritual support.
A medical director, registered nurse, certified nursing assistant, social worker, and chaplain work together to coordinate support for your family. We are here to talk you through your options, answer questions about your concerns, and help you understand what to expect at each stage of a patient's diagnosis.
Our services meet you wherever you consider home
We provide care in homes, assisted living facilities, and nursing homes. We are a friendly face for your loved one, and another set of eyes for you.
Emphasizing education for family and loved ones, we want you to feel secure in your understanding of what to expect, how to manage basic care yourself, and how to contact VNA at any time
Covering other benefits as outlined by your health insurance, such as medical transportation, Durable Medical Equipment (DME), medications and other supplies
Aligning with each family’s needs, we provide grief and bereavement services for a minimum of 13 months
Hospice Inpatient Unit
Our 15-bed facility is now open at PHMC Public Health Campus on Cedar in West Philadelphia, located at 501 S. 54th Street.
VNA care will stabilize patients with difficult-to-manage symptoms in a hospice inpatient unit
Respite services for caregiver relief
24/7 monitoring by nursing staff with daily rounds by a medical director/healthcare provider
Additional support from clinical social workers and spiritual counselors upon request
Amenities include private rooms, family room, and a cafeteria
