Home Health Care (215) 473-7600
Home Health services are available for patients recovering from an illness or for care and self-management education for chronic illnesses. We bring care to your home through a multidisciplinary team of skilled professionals. Nurses, physical therapists, occupational therapists, speech therapists, social workers and home health aides promote recovery; restore health; teach patients and families how to manage chronic illness; and maximize independence in activities of daily living.
Nurses observe and assess your medical condition and may provide help with medications, wounds, tubes, and other treatments. Nurses provide care coordination and education to you and your caregiver. VNA nurses are highly skilled in providing care for acute and chronic illnesses and specialized treatments ordered by your healthcare provider.
Through a comprehensive plan of care developed to address your individualized needs, your VNA team works with you in the comfort of your home. Our team provides the highest quality of care to help you achieve optimal health status.
We offer a variety of supports for recovery working with you to identify a care plan for your physical, mental, and social well-being while in your home.
