Specialty services provided by expert clinicians address the specific needs of patients with a variety of diagnoses and needs such as diabetes, heart disease, stroke, cancer, dementia, wounds, incontinence, ostomies, and mental/behavioral health problems.
Telehealth
- Nursing visits are supplemented with daily monitoring of weight, blood pressure and oxygen levels using touch screen technology. Medical information is sent over the phone lines to the visiting nurse and physician who monitor the results daily.
Long-Term Care of Chronically Ill Patients
- Our visiting nurses evaluate and manage, on a long-term basis, chronically ill patients who have multiple medical and social problems.
