Diabetes Education
  • Provide one on one Diabetic Teaching

  • Trained clinicians educate on healthy eating and how to stay active

  • Assess and teach on medications and how to be independent with medication regimen

  • Educate on preventative care and coping skills

Wound / Ostomy Care
    • Certified Wound, Continence & Ostomy Nurses (WOCNs)Wound Management and Teaching

    • Wound Care Technology to upload images and track healing over time

    • Pre Op Education and Stoma Site Mark-up

    • Post Op Ostomy Management and Teaching

    • Ostomy Fitting

  • Daily monitoring of weight, blood pressure and oxygen levels using touch screen technology are supplemented by nursing visits. Medical information is sent over the phone lines to the visiting nurse and physician who monitor the results daily.

In-Home Palliative Care
  • Provides care to patients with life-limiting illnesses at any stage of illness, whether patient is receiving curative treatments or considering hospice. Our goal is to alleviate suffering through pain control and symptom management measures to improve the patient’s quality of life.

Joint Replacement Program
  • Nursing and Therapy services initiated in 24-48 hours after hospital discharge

  • Anticoagulant and Blood Work Management
