Home Health Care

Home health care services are provided throughout the community to patients of all ages with acute and chronic illnesses. Nurses, physical therapists, occupational therapists, speech therapists, and home health aides promote recovery; restore health; teach patients and families how to manage chronic illness; and maximize independence in activities of daily living. Specialty services provided by expert clinicians address the specific needs of patients with Alzheimer’s Disease, diabetes, heart disease, stroke, cancer, dementia, wounds, incontinence, ostomies, and mental/behavioral health problems.
